|
OVERVIEW: Provides birth and death certificates, as well as vital statistics for the county.
SERVICES:
1) DEATH CERTIFICATES: Issued locally for thirty days following a death at a fee of $18 for the 1st copy and $20 per additional copies payable by cash or check (Do not accept out of area checks). After that time you must send it to the Division of Vital Records, 6550 Reisterstown Road, Baltimore, Maryland 21215 and the fee is $10 for the 1st copy and $10 per additional copies. You must provide the person's name, date of death, and place of death in order to obtain a record. Death certificates are released to a surviving relative or authorized representative.
2) BIRTH CERTIFICATES: On file at the health department from 1940 to present with a cost of $20.00 per record paid by cash or check (Do not accept out of area checks). A birth certificate needed for someone who was born before 1940 must be obtained from the Division of Vital Records, Baltimore, Maryland. The fee is $10.00 when ordering from Baltimore. MUST be listed on the birth certificate AND have a valid, unexpired, State-Issued ID such as a driver's license to make application for a birth certificate.
3) TO CORRECT/ADD NAME: Individual must contact vital records to determine if they meet the requirements for a change and to obtain a copy of affidavit form. Affidavit to be completed and notarized before returning to the department. For children under the age of one year, corrections may be made to given names on the certificates. Upon receipt of the notarized affidavit a corrected birth certificate will be issued. To correct spelling of a name on the birth certificate supporting documents are required.
EXAMPLES of IDENTIFICATION: Department of Motor Vehicles photo identification cards, valid driver's license, United States Passport. NO EXPIRED, CRACKED, CHIPPED, BROKEN, FRAYED, FADED or SIDE VIEW IDs ACCEPTED and ALL IDs must have an issue and expiration date-NO EXCEPTIONS. If your ID does not include a full middle name, you will be required to show a second form of identification. We reserve the right to refuse any ID that we deem questionable. Must be listed on birth record to obtain copy (father, mother or individual).
