Description
|
OVERVIEW: Serves as the lead agency for the Senior Care and Family Caregiver Support Program. Also collaborates with various community and state agencies, nursing homes and adult day care facilities concerning services to disabled elderly.
SERVICES:
1) SENIOR CARE: Provides in-home services and is able to purchase limited supplies for individuals who are 65 and older, who are severely or moderately disabled and meet income and asset guidelines.
2) NATIONAL FAMILY CAREGIVER SUPPORT PROGRAM (NFCSP): Offers respite care for caregivers.
3) NURSE MONITORING: Provides visits to assess need and proper placement while overseeing the agencies providing services.
