Description
|
OVERVIEW: Promotes healthy birth outcomes,positive parenting, parent-child bonding, child health and developmentavailable to expecting moms in their first trimester up to 3 months postpartum.
SERVICES:
1) IN HOME VISITS: Family assessment worker, casemanager provides primarily in home care coordination, linking participants toindividualized services. Pregnant and postpartum women receive information andreferrals for addictions and family planning services. Parents are taught howto cope with their drug exposed newborns.
2) INFANTS and TODDLERS: Newborns to 5 year oldsare evaluated for signs/symptoms of developmental delay, behavioral problemsand other related health issues. If appropriate these children are referred tothe Infants and Toddlers program for therapeutic services or they may continueto be monitored for signs/symptoms of late onset of developmental delay relatedto substance exposure.
3) LEAD POISONING: Educates and provides resourcesconcerning lead poisoning issues.
