|
EVALUATIONS: Provides elderly and disabled adults with free comprehensive evaluations, including medical/nursing, environmental and psychosocial assessments. This evaluation is used to develop a care plan, outlining the services needed to help the individual remain at home, or in the least restricted environment possible.
