|
MEDICAL TRANSIT: Provides transportation to medical appointments for persons receiving Medical Assistance who are unable to use Mobility or the bus. This program provides "last resort" transportation for ambulatory, wheelchair, and stretcher-bound recipients who have no other means available of transport to get to and from medical appointments. Also handles transportation complaints.

Hours Of Operation
|
Call for appointment. Call at least 24-48 hours in advance of appointment for scheduling. If calling for the next day, must call before 3PM. CANNOT do same-day appointments. If calling on Friday, cannot schedule for Saturday.

|

Documents Required
|
Medicaid/Medical Assistance number, Doctor/Provider must complete a Provider Certification form.
