Description
|
CASE MANAGEMENT: Helps Medical Assistance eligible persons, who are chronically ill or disabled and at risk of being institutionalized, remain in their home. A registered nurse/case manage monitors and assess the applicant and develops a plan of care to meet the client's needs. The case monitor coordinates with the recipient a Personal Care Provider who assists the recipient with activities of daily living such as transferring, bathing, dressing, feeding and toileting. Must have a doctor's authorization to qualify for services.
