LONG TERM SERVICES/ CASE MANAGEMENT: Helps individuals over 65, persons with physical and mental disabilities, long term ill children, and individuals eligible for both medicaid and medicare. The services are provided in institutions such as nursing homes, home, and community based settings. The services help individuals remain in their homes and communities that they desire and are able to. The community first choice program make it easy for medicaid participants receive institutional levels of care in home and community based settings. The program provide services that helps the participant feel more comfortable.
