Description CARE COORDINATION SERVICES: Provides outreach and short-term care coordination services to clients that have Medical Assistance in Baltimore City. These services include telephone assessments, education about Medical Assistance and the Managed Care Organization systems, how to use the health benefits, and assistance in connecting to a primary care doctor and community resources. Serves pregnant women, children, individuals with behavioral health diagnoses and chronic health conditions, as well as those that have lost contact with their medical provider.

Maternal Home Health Visiting: Offers a Central Intake and Triage System linking families to in-home health education and social services. Provides connection to WIC and other needed resources, as well as linkage to long term home visiting services for pregnant women and infants through B'More for Healthy Babies Home Visiting Services.  Nurse-Family Partnership is a home visiting program that supports moms during pregnancy and after birth, teach about child development, make referrals for services (healthcare, job training, childcare, birth control, mental health), help with setting goals for self and family.

Safe Sleep Program: Provides education about 'safe sleep' practices, as well as a pack n' play (portable crib) to families who do not have a safe sleep environment for their baby.

​Provides Ombudsman services to act as a Medicaid recipient advocate and help with resolving complex medical issues.

Assists with removing barriers to healthcare by linking to housing, domestic violence services, Energy Assistance, and Medical Assistance Transportation resources. 

HealthCare Access Maryland and the Division of Parole and Probation are working together to provide advocacy and outreach for individuals being released from incarceration regarding health insurance.
