Description
|
INFORMATION AND REFERRAL: The LCT is an interagency team of representatives from local child and family serving agencies, as well as representatives of family members. The LCT provides a forum for families of children with intensive needs to receive assistance with the identification of and referral to potential resources to meet identified needs. LCT meets 1st and 3rd Thursdays of each month.

Eligibility
|
Baltimore County families with children in need of assistance regarding behavioral health.

Program Fees
|
None.

Application Process
|
Family members, school faculty, and community members can make referrals directly to the
Local Care Team, and youth ages 18 to 21 can self-refer. Complete the referral form and
submit by email. A letter describing the youth's current level of care/treatment
recommendations as well as barriers experienced may also be helpful. The youth's provider
can assist with this form’s completion.
