Description
|
Provides two transportation programs, the Mobility Management Program (MMP) and Reach Your Destination Easily (RYDE).
MMP: Provides free transportation to dining program participants to access meal sites. Offers monthly bus passes, gas cards, and paratransit rides. Click here to fill out the Mobility Benefits Application.
RYDE: Provides a curb-to-curb transportation and local trip planning service. Offers transportation 8 miles from a participant’s home for non-medical trips, and up to 16 miles from a participant’s home for medical trips. Also provides trips to the Sunnyvale Caltrain station and the Veterans Hospital in Palo Alto regardless of distance from the participant’s home. Offers local transportation to appointments, grocery shopping, or visits with friends.

Eligibility
|
Mobility Management Plan (MMP):
-Reside in Santa Clara County
-Complete a new Senior Nutrition Program(SNP) registration form
-Qualify as low-income based on the Elder Economic Security Index(EESI) for Santa Clara County
-Need the transportation benefit to be able to attend the meal program
-Bus passes and gas cards require regular dining site attendance
-Paratransit participants must be certified paratransit eligible by the Santa Clara Valley Transportation Authority
RYDE:
-Must reside in Santa Clara County
-Must be 65 years of age or older, and ambulatory (able to walk on their own, or be able to walk assisted with a cane or walker).
-Participants are allowed only one companion per passenger

RYDE:
To request a ride, call 2 business days in advance. Rides can be booked up to 4 weeks ahead.
Residents living in the following zip codes 95110, 95112, 95116, 95118, 95119, 95122, 95123, 95139 can schedule must call RYDE number or email.
