Long-term case management designed to help families take action toward self-sufficiency. - Participants meet with a case manager to develop a personalized action plan to achieve self-determined goals.
- Role of the case manager is to be a support system to empower participants to reach self-sufficiency.
- Participants and case managers track and celebrate progress along the way, meeting potentially once per week.
