Healthy Neighborhoods Program (HNP) - provides FREE home safety visits and distribution of supplies to promote healthy homes.
Community Health Workers - dedicated staff to provide on-going support and connection to resources to address overall well being. - promotes healthy eating, physical activity and avoiding tobacco to reduce risks for cardiovascular disease, obesity, diabetes, COPD, asthma and cancer
- advocates for policy, organizational and environmental change to support individual behavior change
- partners with local business, media, health providers, schools, legislators and community agencies to achieve these objectives
- conducts a Community Health Assessment, works with stakeholders to develop and promote a Community Health Improvement Plan

- Harmonicas for Health- COPD and asthma
