Individuals can apply for Medicaid, Child Health Plus, the Essential Plan or a Qualified Health Plan in three ways: - directly contact Certified Application Counselors (see Molina and Fidelis Care entries for local contacts)
- through a Health Access Navigator, such as those at the Human Services Coalition
- Online at www.nystateofhealth.ny.gov
