Description
Provides intensive family-based service coordination for children who have a severe emotional disability (SED), have a substance use disorder, or have a co-occurring chronic physical health problem. Provides services to the child/youth and their family. Serves the Seven Counties service region including Jefferson, Bullitt, Henry, Oldham, Shelby, Spencer, and Trimble counties.  The mission of the program is to assist the children with living in the community and reduce the incidence of hospitalization and placement.  The Service Coordinators do not provide direct clinical service; however, they are in close contact with the providers and their role is to coordinate communication and service efforts. The Primary function is targeted case management to be able to assist families securing the resources needed.

A diagnosis of Severe Emotional Disorder is made with a thought, mood, perception, orientation, memory or behavior disorder ongoing for at least one year and disrupting certain areas of functioning including self care, interpersonal relationships, family life, self-direction, or education.
