Description
Provides outreach and case management services to adults in the Louisville area who are homeless and mentally ill and assists them in obtaining transitional or permanent housing and connecting with mental health services, as part of Seven Counties' Center for Supported Living division. Case managers can continue to work with the client for up to six months after client obtains permanent housing. Unable to provide services to those who are living with someone else, even if their name is not on the lease.
