Program Delivery
Eligibility: Older adult who is 60 years of age or older and receives Medi-Cal benefits with an eligible Aid Code.
Needs assistance to safely perform 3 or more activities of daily living and, who is at-risk of nursing home placement. Participants must be amenable to care management.
Languages: Language interpreting services available upon request, American Sign Language, Arabic, Hmong, Spanish, Tagalog, English

Program Fees: Accepts Medi-Cal.
Documents Required: Medi-Cal card and/or Social Security card.
Normal Wait Time: Varies depending on wait list length, generally 1 - 4 months.
Service Area: Serves all of El Dorado, Sacramento, and Yuba Counties; from Colfax and below in Placer County, areas between Sacramento and Yolo Counties in Sutter County, and the loop from West Sacramento, Davis, Winters, and Woodland, and between Sutter and/or Yuba Counties.
