ADULT SOCIAL SERVICES: Provides case management to adults who have functional disabilities due to advanced age (65+), without regard to income. Adults under age 65 who have disabilities may qualify, provided their income does not exceed 80% of the State median income. Case management activities include a functionality assessment, referrals to other helpful agencies, and continuous evaluation of the client's situation to help them to continue to live independently.
