OVERVIEW: Provides assistance with screening and applying for Medical Assistance/Medicaid/MA programs:

1) AGED, BLIND, DISABLED, MEDICALLY NEEDY: Serves individuals ages 65+. Serves disabled individuals under the age of 65 who are blind, physically or developmentally disabled.

2) QUALIFIED MEDICARE BENEFICIARY (QMB): Serves individuals who are on Medicare. Helps pay for Part A and Part B premiums, co-payments, and deductibles. For questions/concerns, call Medicare at 800-633-4227.

3) SPECIFIED LOW-INCOME MEDICARE BENEFICIARIES (SLMB): Serves Medicare recipients who are ineligible for Qualified Medicare Beneficiary Program. Pays only Part B premium. Includes $20 per month income disregard. For questions/concerns, call Medicare at 800-633-4227.

4) EXPANDED MEDICAL ASSISTANCE: Eligibility is based on income limits. For Medicaid recipients who have questions/concerns, call the MA Helpline at 800-456-8900. Uninsured individuals should call Frederick County DSS, or Maryland Health Connection at 855-642-8572.

5) LONG TERM CARE MEDICAL ASSISTANCE: Medical insurance for clients who are residing in a long term care facility for more than 30 days.

  • For families, children, and pregnant women apply at Marketplace: marylandhealthconnection.gov
  • To request paper applications: visit the Frederick County Department of Social Services (FCDSS) office, download from the State of Maryland's Department of Human Services (DHS) website, call the DHS Call Center at 1-800-332-6347, or email [email protected].
  • To submit paper applications: email to [email protected]; fax to 301-600-2663; or mail to PO Box 237, Frederick MD 21705. Return in-person at DSS lobby or drop-box.
  • Walk-in hours for the Medical Assistance Lab at DSS are Monday-Friday, 9AM-11AM.
