Description

EARLY INTERVENTION: Provides early intervention services, including case management, developmental evaluations, audiology, physical, occupational, and speech-language therapy, health/diagnostic screenings, nutrition counseling, and family health. Also provides referrals to parent support groups. Offers a family support network for any parent whose child is a part of the program. Notifies parents of conferences and workshops.

Program Delivery
Eligibility: Children ages 0-3 and their families/caregivers. Child has a 25% delay in at least one or more of five developmental areas (cognitive; physical, including vision and hearing; communication; social or emotional; adaptive); OR manifests atypical development or behavior in one or more of the five developmental areas, interferes with current development, and is likely to result in a subsequent delay (even when diagnostic instruments and procedures do not document a 25% delay); OR has a diagnosed physical or mental condition that has a high probability of resulting in developmental delay. Children
Languages: Language translation services available., English
Application Process: Accepts referrals from a parent, grandparent, friend, other relative, healthcare or education provider, child care or social service provider, or a staff person from the NICU or hospital. Referrals must be completed on the state website at https://referral.mditp.org/.
