Description
HOUSING: Provides supportive housing, personal care and case management services to single elderly, persons with AIDS or disabled adults who cannot live alone primarily due to mental or physical health problems. Placements are in private homes. Providers supervise residents in meeting their physical and social needs and arrange for transportation to medical appointments and social activities.
