Program provides linkage to housing and community supports for individuals and families that are homeless. Provides up to a year of ongoing supports to help individuals and families maintain stable housing.

Program Delivery
Eligibility: Individuals must be living in an emergency shelter or with no fixed, adequate nighttime residence. With families head of household must be homeless (such as living in a shelter, fleeing domestic violence or living in a car)
