Eligibility: Connecticut resident eligible for one of the following low income programs: SNAP, TFA, Husky, State Supplement, C4K
Application Process: Must complete Low Income Pet Sterilization Application (available online) and DSS Permission to Determine Eligibility form
Program Fees: No fee
