Description
Provides care coordination works to coordinate services for children ages 0 through 18 (or 21 if still receiving services from a local educational authority) who have complex behavioral health needs. Care Coordinators work in partnership with families using the wrap around process to broker and advocate for services, and to coordinate and monitor the implementation of an Individual Service Plan for children displaying serious emotional and behavioral disturbance who are at risk to be, or have already been, separated from their family and/or community for the primary purpose of receiving mental health or related services.

Program Delivery
Eligibility: Age 0 through 18 (or 21 if still receiving services from a local educational authority) who are displaying serious emotional disturbances (SED). SED refers to children and adolescents with a mental, behavioral, or emotional disorder that has resulted in functional impairment that substantially interferes with or limits the child's functioning in family, school, or community.
Languages: Spanish, Portugese, Creole, Portuguese
Application Process: Self referral or agency referral; Families must be involved in seeking services
Program Fees: No fee for care coordination; Care coordinators will help families access insurance if the family is uninsured
