Description
Reconnecting Families Services engages, supports and intervenes with family members through a short-term, intensive, in-home service model following the removal of a child(ren) due to abuse or neglect. The operating hours are at times convenient to the family and the frequency of hours per week are based on family/case circumstances. There is a minimum of one (1) home visit per week prior to reunification, 2 visits weekly in the first 30 days after reunification and 1 visit per week after that. The family will be linked to appropriate services in the community prior to the conclusion of this service.
