Eligibility:

Ages 0 through 18 (or 21, if still receiving services from a local educational authority) who have complex behavioral health needs; priority is given to children and youth with behavioral health needs who are at imminent risk for placement in residential or hospital level treatment or who are returning from residential or hospital level treatment

Self referral or agency referral; Families must be involved in seeking services

|
Program Fees:
|
No fee for case management; Families use their health insurance to pay for services; Case managers will help families access insurance if the family is uninsured
