Description
System of Care/Community Collaborative provides care coordination for children ages 0 through 18 (or 21, if still receiving services from a local educational authority) who have complex behavioral health needs. Care Coordinators work in partnership with families to broker and advocate for services, and to coordinate and monitor the implementation of an Individual Service Plan for children displaying serious emotional and behavioral disturbance who are at risk to be, or have already been, separated from their family and/or community for the primary purpose of receiving mental health or related services.

Program Delivery
Eligibility: Ages 0 through 18 (or 21, if still receiving services from a local educational authority) who have complex behavioral health needs; priority is given to children and youth with behavioral health needs who are at imminent risk for placement in residential or hospital level treatment or who are returning from residential or hospital level treatment
Application Process: Self referral or agency referral; Families must be involved in seeking services
Program Fees: No fee for case management; Families use their health insurance to pay for services; Case managers will help families access insurance if the family is uninsured
