Program Delivery
Eligibility: Patients who have exhausted benefits or all third party coverage, or who are not eligible for such coverage, (such as private insurance, Medicaid, Medicare, coinsurance, etc.) and who cannot afford to pay for cost of treatment. Eligibility is based on the U.S. Department of Health and Human Services Income Guidelines.
Application Process: Must complete application form and bring in required documents
Documents Required: Income verification applies to all family members residing at applicant's legal address: Last Income Tax Return Filed, if self employed; Statement of wages from Unemployment Compensation showing date unemployment started, if applicable; Payroll stubs for the last eight consecutive weeks; If there is no income for the last eight weeks, a notarized letter stating that no income has been received in the last eight weeks is required
