Eligibility:
|
Age 16+; Eligible patients are medically stable, will benefit from 24-hour physician availability, require a combination of physical, occupational, and/or speech therapy, are able to tolerate and benefit from intensive therapy, and have a realistic discharge plan

Referrals from physicians, discharge planners, case managers, healthcare professionals, insurance representatives, families and patients
|

|
|
Program Fees:
|
Financial counseling is available for patients with no medical coverage
|
