Description
Community case management services for individuals with mental illness, substance abuse, HIV/AIDS, who have been released from incarceration or who are homeless. Goal is to assist the individual in skill building and to live independently in the community. Services may also include peer support, self advocacy, service plan development, assistance in obtaining and maintaining housing, budgeting, linkage to employment supports, assistance in accessing entitlements, and assistance in accessing medical, substance abuse, and mental health services.
