Eligibility:

91 Northwest Drive Location: Resident of Avon, Berlin, Bristol, Burlington, Canton, Farmington, Plainville, Plymouth, Simsbury, or Southington; 43 Woodland Street Location: Resident of Hartford or West Hartford; Ages Birth through age 18 (or age 21, if still receiving services from a local educational authority) who have complex behavioral health needs; Priority is given to children and youth with behavioral health needs who are at imminent risk for placement in residential or hospital level treatment or who are returning from residential or hospital level treatment
