Description
Program funded by Connecticut State Department of Mental Health and Addiction Services provides case management for SAGA Cash recipients, Medicaid LIA and medically indigent clients. Participants are usually people who frequently use acute services such as detox, and who are known to the treatment providers as chronic relapsers. Case managers provide linkage to treatment services, assistance with problem solving, advocacy for entitlements and housing, and other supports designed to improve quality of life in order to reduce incidence of relapse.
