Requires proof of identity and proof of residency; HOW TO OBTAIN AN ID WITH LITTLE OR NO ID Get a copy of your medical records from your health provider or by visiting the Medical Center South (100 Church Street South, Suite 300) or call 203-688-2231 for assistance. Follow the steps to get your Social Security Card, With your social security card and your medical insurance card you will be able to request your Birth Certificate, If you do not have medical insurance, then fill-out the voter registration application online and wait for the voter registration card to arrive in the mail. Follow the steps to get your Birth Certificate, You will be able to secure a DMV ID with your Social Security Card and your Birth Certificate plus two (2) pieces of mail from two (2) different sources, Follow the steps to get our DMV ID
ACCEPTABLE FORMS FOR PROOF OF IDENTITY ONE of the following: *Valid Passport *U.S. Driver's License (current) *Birth Certificate (for children 16 and under only; others must also present a photo ID) *Consular ID Card (current)
OR TWO of the following *National identification card (must have photo, name, address, date of birth and expiration date) *Foreign driver's license (current - NOT expired) *Voter registration card (must be presented with original birth certificate) *Military identification card (U.S. or foreign) (must be presented with original birth certificate) *Visa (current) *ITIN card (must be accompanied by an ID with a photograph)

Two proofs of residency must come from two different places. Residency documents must show the name and current address of the applicant. Residency documents in the name of only one spouse will be accepted for married couples only if an original marriage certificate is presented.
