Description
Intensive community based transitioning services are provided to young adults ages 18 through 25 who are struggling with mental illness. Focus is on helping young adults learn skills they need to live independently in the community. Services include case management, outreach therapy, medication evaluation, and management and assistance with employment and education goals.
