Project Fresh Start works with community partners, state agencies and other reentry organizations to support the reintegration of formerly incarcerated residents into the New Haven community.

Program Delivery
Eligibility: Formerly incarcerated residents into the New Haven community
Application Process: HOW TO OBTAIN AN ID WITH LITTLE OR NO ID Get a copy of your medical records from your health provider or by visiting the Medical Center South (100 Church Street South, Suite 300) or call 203-688-2231 for assistance, Follow the steps to get your Social Security Card; With your social security card and your medical insurance card you will be able to request your Birth Certificate; If you do not have medical insurance, then fill-out the voter registration application online and wait for the voter registration card to arrive in the mail, Follow the steps to get your Birth Certificate, You will be able to secure a DMV ID with your Social Security Card and your Birth Certificate plus two (2) pieces of mail from two (2) different sources, Follow the steps to get our DMV ID
