Program: Case Management for Chronic Relapsers

Agency: WELLMORE BEHAVIORAL HEALTH

Resource Number: 17361387

Pilot program funded by Connecticut State Department of Mental Health and Addiction Services provides case management for chronic relapse substance abusers. Participants chosen for the pilot are usually people who frequently use acute services such as detox, and who are known to the treatment providers as chronic relapsers.
