Eligibility:
|
Ages 18+; Two or more admissions to detox or inpatient rehab. within the last 12 months, or frequent use of community resources (i.e. ER, police, shelter); Meet the DSM IV criteria for substance dependence; At least 3 of the following: 1. History of needing continuous supportive treatment; 2. Current housing or homelessness increases risk of relapse; 3. Lack of financial resources; 4. Other condition(s) increases risk of relapse; 5. Involved in or appropriate for specialized services, i.e. DCF, AIC, probation.
