Eligibility: Injured workers
Application Process: Application for services may be submitted as soon as it is apparent the applicant may not be able to return to their regular work; Self-referral, or referral by any interested party are all acceptable; Applicant does not have to wait until he or she has reached maximum medical improvement to apply; Contact Rehab Services at the Central Office or download an application form from the agency website
