Wheeler Family Health & Wellness Center, One Hope Street, Bristol offers medical care for adults, children and families; physical exams; pediatrics, COVID and flu vaccines, chronic disease management for asthma, diabetes, hypertension/heart health, HIV, and treatment and services for Hepatitis C; Women’s Health and preventive health screening for women, annual wellness visits, gynecology, and family planning, as well as parenting supports and infant and toddler nutrition; and chiropractic care. Offers behavioral health services for all ages, including individual, group, and family therapy to help with anxiety and depression, defiant behaviors, trauma, grief, substance use disorders; an Intensive Outpatient Program for adolescents (12 to 18) and adults; and more. Psychiatric services, including evaluations and medication management, are also provided. Same day walk-in access for behavioral health services. First-time evaluations are conducted Monday–Friday: 9am–3:30pm. Bristol Health lab accepts lab slips from Wheeler Family Health and Wellness Center providers. Services are LGBTQ-responsive and are offered in English and Spanish.

Additional services include:

• Eye movement desensitization and reprocessing (EMDR) services available.

• Treatment for addiction, including individual and group therapy and medications for substance use disorder such as extended-release naltrexone (Vivitrol) and buprenorphine (Brixadi, Suboxone and Sublocade - buprenorphine extended-release) for alcohol, heroin and opioid addiction.

• Peer support staff in addiction treatment help patients through their common lived experiences.

• Care management services, including coordination with outside services.

• Community Health Workers, who can connect patients to services and resources in the community.

• Support for the basic needs of life.

• Nutrition Services: Nutrition counseling, self-management of diabetes and other health conditions, pediatric nutrition program, and eating for healthy lifestyles counseling.


Wheeler Family Health & Wellness Center, Hartford offers primary care for all ages, well-child care and pediatrics, women's health care, complementary medicine, behavioral health services, treatments for depression, dental care and coordination, treatment for addiction, including medication-assisted treatment for alcohol and opioid use, treatment for depression tobacco cessation, outpatient services, geriatric services, nutrition counseling, care management, HIV case management and recovery supports. Other services include the Bettor Choice Gambling Treatment program, Substance abuse treatment includes medication-assisted treatment (such as extended-release naltrexone (Vivitrol) and buprenorphine (Suboxone) for alcohol, heroin and opioid addiction, tobacco cessation services and recovery supports, and behavioral health services. Walk-in services, same-day care provided. Services are available in both English and Spanish.

Medicaid, Medicare, and commercial health insurance welcome. A sliding fee scale is available based on family size and income. No patient will be denied health care services due to an individual’s inability to pay for services.

Walk-In Access to Behavioral Health Services: Monday–Friday: 9am–3:30pm

  • Walk-in care is available on a first-come, first-serve basis. Walk-in appointments are based on staffing availability at the time.

Wheeler outpatient and primary care sites, including Family Health & Wellness Centers in Bristol, Hartford, New Britain, Plainville, and Waterbury operate from 8am-6pm during the week.

  • Individuals that have a medical need that requires assistance prior to the next business day, should call the Navigation Center at 860-793-3500 or Toll free at 888-793-3500. Individuals should leave contact information and a member of the medical staff will return the call. In the event of a medical emergency, call 9-1-1.
