Description
There are two Seven Counties Services ACT teams:

1. Metro-CABHI "Cooperative Agreement to Benefit Homeless Individuals" ACT Service Area: Jefferson County, KY  Address: 708 Magazine St, Louisville, KY 40203
2. Rural ACT Service Area: Bullitt, Henry, Oldham, Shelby, Spencer, and Trimble Counties in Kentucky  Address: 4710 Champions Trace Lane, # 104, Louisville, KY 40218 (based in Jefferson County, but serves the above counties)

Both teams offer the same services and have essentially the same criteria.
 
The ACT teams provide case management, individual and group therapy, nursing services, medication management, psychiatric services, supported employment, peer support and payee services to adults with severe mental illness who have already tried and can no longer access/benefit from typical outpatient services and do not have an existing support system to help prevent hospitalization, homelessness or placement in a personal care home. Referrals to this program typically come from state guardians or other Seven Counties programs, who have tried working with the person, and he/she needed more intensive help. Services are delivered in the context and environment where they are needed rather than in a traditional office setting. Services are flexible and available 5-days per week (on-call available for crisis situations 24/7). Individuals cannot self-refer for these services.
