Provides permanent supportive housing assistance for disabled homeless individuals and their partner. Central Valley Housing does not provide housing. Eligible household can search for housing throughout San Joaquin County. The cost of housing must meet HUD guidelines.

Program Delivery
Eligibility: Meet all criteria:

-Have a permanent disabled condition

-Experiencing homelessness such as living in an emergency shelter (or similar location) or a place not meant for human habitation (car, street, tent, abandon building)

-Be homeless continuously for the past 12 months or had at least 4 separate homeless periods in the past 3 years adding up to 12 months

-Meet income guidelines, and be willing to develop a plan for self-sufficiency

-Have evidence of residency in San Joaquin County
Languages: English and Spanish. Additional languages are provided via a translation service.
Application Process: By referral only through the Coordinated Entry System (CES). Call 211 San Joaquin for screening and intake Monday-Friday 9am-4pm.
Program Fees: No fees
Documents Required: Proof of residency in San Joaquin County, documentation of homelessness and disability, verification of income and ID for both members
