Program Delivery

Eligibility:	Meet eligibility criteria: - Single individuals with permanent disabilities. - Those experiencing homelessness, living in an emergency or homeless shelter, car, street, tent, or an abandoned building. - Homeless for 12 continuous months or 4 separate periods in 3 years totaling 12 months. - Individuals meeting income guidelines. - Those able to share housing and care for themselves. - Residents of San Joaquin County with proof of residency.

Languages:	Additional languages are provided via a translation service, Spanish, English
Application Process:	By referral only through the Coordinated Entry System (CES). Call 211 San Joaquin for screening and intake Monday-Friday 9am-4pm.

Program Fees:	No fees
Documents Required:	Proof of residency in San Joaquin County, homelessness and disability documentation, income verification, and Photo ID.
