VITAL RECORDS:

  • DEATH CERTIFICATES: Issued by State of Maryland since 2015 following a death at a fee of $20 payable by credit card, cash, or company check. For death certificates issued prior to 2015 you must send to the Maryland Department of Health's Division of Vital Records in Baltimore. The fee is $10 for the first copy and $12 for each additional certificate if purchased on the same day. Applications can be downloaded from the website.
  • BIRTH CERTIFICATES: On file and available same day at the health department from 1940 to present with a cost of $20 per record paid by cash or credit card. Must have acceptable photo identification and be one of the three persons listed on the birth certificate. A birth certificate needed for someone who was born before 1940 must be obtained by mail from the Maryland Department of Health, Division of Vital Records in Baltimore, Maryland. The fee is $10.00 paid by check or money order payable to Division of Vital Records. Applications can be downloaded from the website.
  • TO CORRECT/ADD NAME: Individuals must contact Maryland Department of Health, Division of Vital Records located in Baltimore, Maryland.

Application Process: Birth certificates and death certificates must be requested in person. Applications are available online.

Documents Required: Death Certificate: Proof you are a surviving relative or authorized representative.
Birth Certificates: Department of Motor Vehicles photo identification cards, valid driver's license, United States Passport. NO EXPIRED, CRACKED, CHIPPED, BROKEN, FRAYED, FADED or SIDE VIEW IDs ACCEPTED and ALL IDs must have an issue and expiration date-NO EXCEPTIONS. We reserve the right to refuse any ID that we deem questionable. Must be listed on birth record to obtain copy (father, mother or individual).
