Description

COVID-19 Update: Services available by appointment only. For assistance call 209-381-1010.

Provides social and health case management for frail elderly clients who are certifiable for placement in a nursing facility but who wish to remain in the community.

The services that MSSP clients may utilize include:

  • Case Management
  • Adult Social Day Care
  • Chore and Personal Care Assistance
  • Respite Care
  • Transportation
  • Meal Services
  • Protective Services
  • Communication Services
