Description
Provides services to transition-age-youth (ages 16-25) who are involved in mental health care or are at risk for being involved in the mental health care system. Offers a community environment and access to peer support specialists who provide support & assistance in the following areas: accessing services, goal-setting, action-planning. 

Offers services regardless of insurance or residence. However, some services (therapy, medication management and case management) require Medicaid. TAYLRD staff can assist individuals with obtaining Medicaid if needed. Individuals can walk in during drop-in hours. Offers case managment and connects clients to therapy, medication management, education, and vocational assistance.

There are two locations for this program:

• Jefferson County location: 1020 East Broadway, Louisville KY, 40204
• Spencer County location: 103 Settlers Center Road, Taylorsville KY, 40071
