Description
Provides one-on-one home visits that last 1 1/2 hours. The home educator works with parents to help the child develop a positive self concept, self confidence, social skills and a love for learning. Home Base provides the same component services as the center-based program.

Eligibility: Must meet income guidelines.

Priority enrollment give to children with an identified disability.

Documents Required: Proof of Income; Birth certificate or proof of custody for all children in the family under age 18; if applicable, current copy of child's IEP/IFSP; child's current annual physical exam; child's current annual dental exam; child's immunization record; child's TB test with results
