Wisewoman services teach women ages 35-64 years who have little or no health insurance how to eat healthy, be more physically active, and quit smoking to prevent heart disease. Services may include blood pressure and cholesterol screening, testing for diabetes and osteoporosis, and lifestyle programs that target poor nutrition, physical inactivity, and smoking.

Eligibility: Women ages 35-64; Woman must be enrolled in the Breast and Cervical Cancer Early Detection Program; Income at or below 250% of the federal poverty level; Have no health insurance; Have an insurance deductible of $1,000 or more; Have health insurance that excludes routine blood pressure screening, lipid profile, and blood glucose screening
