Program Delivery
Eligibility: Adults with chronic mental illness, who are assigned to one of the 17 service teams in Alameda County and have current Medi-Cal.
Languages: French, Laotian, Portuguese, Spanish, English
Application Process: Call to apply. Individuals need to either self-refer and or be referred by their case manager from their service team. Please contact the site for a referral form.
