Description
A comprehensive care management program designed to enable frail seniors to remain safe and independent in own home. Services available include:
  • Personal care
  • Homemaker services
  • Emergency Response System
  • Referral to home-delivered meals
  • Respite care
  • Referrals to medical care
  • Non-medical home equipment
  • Referrals to advocacy counseling
  • Referrals to legal services
  • Referrals to money management
  • A visit by a social worker and a nurse to evaluate senior in their home
  • A care plan development

Program Delivery
Eligibility: Open to seniors who are 65 or older, frail or disabled who:

Live in Oakland/Emeryville/Piedmont or Berkeley zip codes 94704 or 94705

Receive monthly Medi-Cal benefits

Are at risk of nursing home placement because of frailty or inability to manage without help

Are willing and able to accept care management
Languages: Cantonese, Mandarin, Spanish, Tagalog, Vietnamese, English
Application Process: Visit during program hours, call, or visit website to download application, and fax or mail to office.
