Field-based initiative providing specialized outpatient mental health services for older adults in Sacramento County who are experiencing functional impairments due to chronic mental health symptoms.

Services include outreach, mental health rehabilitation and therapy, psychiatric medication management and follow-up, housing support, case management, and more. Service frequency is tailored to each individual's needs, with various services offered weekly.

Program: Seven days a week, 8 am - 5 pm

Program Delivery

Eligibility:	Must meet all of the following requirements: - Must be a Sacramento County resident aged 55 or older. - Must be authorized by the Sacramento County Intensive Placement Team. - Must be returning to the community from acute, subacute, or long-term care settings OR require more intensive services than their current outpatient settings. - Must be receiving or eligible for Medi-Cal.
