Eligibility: Homeless families in which one adult has a disability. Families
Languages: English
Application Process: Entry into Permanent Supportive Housing communities is through the Coordinated Access System (CAS). Individuals who are literally homeless can call 211 Sacramento to complete a Housing Conversation Tool (formerly known as the VI-SPDAT), which will be entered into the Homeless Management Information System (HMIS). Sacramento Steps Forward will match assessed households to available housing opportunities within the Continuum of Care.
