Provides services and support to individuals and families who have moved from temporary housing into their own homes. Provides case management services to clients who live in permanent supported housing.

Program Delivery
Eligibility: Homeless individuals and families. Must have a Shelter Plus Care Voucher or must be in a Project-Based Voucher unit that is not being supported by another agency.
Languages: Spanish, English
Application Process: Entry into the Building Bridges Program is through the Coordinated Access System (CAS). Individuals who are literally homeless can call 211 Sacramento to complete a Housing Conversation Tool (formerly known as the VI-SPDAT), which will be entered into the Homeless Management Information System (HMIS). Sacramento Steps Forward will match assessed households to available housing opportunities within the Continuum of Care.
