Eligibility:

Dignity Health patients who have recently been discharged from the hospital and need short-term skilled nursing, physical therapy, or speech therapy. Individuals are eligible for home health services if they:
- An ill or frail elderly individual
- A mother of a child under 18
- An infant
- A child with special needs
