Program Delivery
Eligibility: Open to Sacramento County adult residents age 18 and older, with psychiatric disabilities. Must be Medi-Cal eligible and meet the criteria as determined by the Sacramento County Department of Behavioral Health Services. These are candidates residing in long-term psychiatric hospital settings with a need for support upon release and others that demonstrate need for more intensive services.
Languages: Most languages provided through interpreter agency services. American Sign Language Interpreter available on request, English
Application Process: Referral required from the Sacramento County Access Team.
