Program Delivery
Eligibility: Sacramento County residents aged 18 and older with psychiatric disabilities. Must be Medi-Cal eligible and meet the criteria determined by the Sacramento County Department of Behavioral Health Services. This includes candidates residing in long-term psychiatric hospital settings with a need for support upon release and others who demonstrate a need for more intensive services.
Languages: Interpreter available upon request, American Sign Language Interpreter available on request, English
